Evergreen Enrollment Process

In order to attend Evergreen Middle School you must live in our residency area. Please call (425)385-4144 to determine which school your child will be attending.

The following forms are required to enroll your child:

  1. Proof of Residency (The legal guardian must provide proof of residency before enrolling their child at Evergreen Middle School. The proof of residency must contain the legal guardian's name and physical address. A lease agreement, water bill or gas bill are a few examples of acceptable proofs of residency)
  2. Request for Records (I will send this to your child's previous school so that your child's records can be sent to Evergreen)
  3. Student Scheduling Information (Completing this information will ensure that your child is placed in the correct classes)
  4. Student Housing Form
  5. Home Language Survey
  6. Enrollment Form (Available in multiple languages)
  7. Immunization Form
  8. Annual Health History
  9. Lunch Application (We will provide this when you come in)
